2012 Camp Wakonda Registration
GENERAL INFORMATION ON REGISTRATION
Registration for Camp Wakonda’s three overnight summer camps is on a first-come first serve basis. Once each camp reaches capacity, registrations will be closed. For a camper to be considered registered, a complete application must be received, as well as the required payment.

A complete application includes:
2012 Health & Medication Form (paper only; signed by parent or guardian)
Camp Wakonda Rules of Conduct (paper only; signed by parent or guardian and camper)
2012 Camp Wakonda Registration Form (can be done on-line or submitted on paper (hard copy)
A payment of at least half of the camp fee must accompany the application. The balance is due at check-in.
 
REGISTRATION DEADLINE DATES
Complete applications must be received by each camp’s deadline date. These dates are:
Junior High Camp – June 29, 2012
Intermediate Camp – July 6, 2012
Senior High Camp – July 13, 2012
No applications will be accepted after each camp’s deadline date. All required forms must be received by the deadline, along with the required payment of at least one-half of the camp fee. Only the balance of fees will be accepted at check-in.
 
WHERE TO SEND FORMS & FEES
Some forms can be submitted on-line. Those requiring signatures must be submitted in hard copy.
Make checks payable to Milton SDB Church.
Payment can be made on-line via PayPal through June 9, 2012.

1. Camp Fee ($150 for Intermediate, $175 for Junior or Senior High)  $__________

YES If yes, enter 0 and proceed to number 3.
 
NO If no, subtract $100 from the fee for Intermediate Camp or $125 from the fee for Junior and Senior High Camp

FOLLOW STEPS BELOW TO ENSURE ALL REQUIRED FORMS ARE PROPERLY FILLED OUT.
Applications will not be considered complete until all forms and fees are received.
STEP 1: Print and completely fill out the 2012 Health & Medication Form.
STEP 2: Print and completely fill out the Camp Wakonda Rules of Conduct Form.
STEP 4:
               If you are NOT registering online: Print and completely fill out the 2012 Camp Wakonda Registration Form.
               For online registration: Fill in and submit the information below. No paper copy of the 2012 Camp Wakonda Registration Form needs to be submitted.
STEP 5: Mail in all printed and completed forms to the Milton Seventh Day Baptist Church also include payment if not applying online.
